Team Acceptance
The Entry fee must be paid and the roster (including the email of the head coach and team rep) must be complete on AES for acceptance.
Event Roster & Medical/Emergency Release Form Requirements: 1. All players listed on the roster must be registered or members in good standing with their respective member organization. 2. All players must meet age classifications requirements. NOTE: Age waiver players are NOT eligible for qualification events and national competitions (National & Regional Qualifiers and the Junior Olympics) 3. All staff listed on the roster must be registered or members in good standing with their respective member organization. A staff member listed on the roster for the team/club will be with the team/club at all times during while attending this competition. 4. All coaches are required to be impact certified at a minimum. 5. A staff member listed on the roster for the team will be with this team and have in their immediate possession during this competition a complete and legible copy of the medical/emergency release form for each player listed on the official roster. 6. The team understands it is subject to any and all penalties for incorrect or incomplete information on this form or incomplete or incorrect results information. 7. The roster for any team earning a bid will be frozen as submitted on this form.

Checkin Information:
The Club Director and/or Coach will be required to present 2 Clean AES rosters at checkin. Any changes can be made on provided computer terminal and printer for a fee. This must be done prior to Team play. Please do this carefully. No Roster additions (players or coaches) may be made at a QualifierTournament once a team has commenced match play in that event. Late arriving teams will be subject to sanctions per current USAV Official Rule Book.
NO COACHES MEETING WILL BE HELD.
Medical:Athletic trainers will be available throughout the tournament.
Please be sure to bring your own medical kits/first aid equipment or be prepared to purchase supplies. Standard emergency protocols will be followed in case of injury. Coaches, you are required to have your USAV Medical Release forms with you at all times during the event.
Tournament and athletic apparel will be available onsite through Lucky Dog and tbd.
Concessions:
Concessions will be sold. Please be sure to adhere to all food/beverage restrictions at each site. Absolutely NO carry-in food (coolers) are allowed anywhere in the facility. Team's that violate any of the facilities rules may be sanctioned.

- Check-In: McCormick Place South bldg Hall A: Friday at 6:30pm to 9:30pm
- Saturday checkin is at Hall A 6:45am to 12:30pm.
